Exosomes Hair Regeneration

Introduction

At Ageless Medical Spa and Hair Restoration in Mount Pleasant, SC, we are committed to offering cutting-edge solutions for hair loss. Exosome therapy is one of our most advanced treatments, harnessing the regenerative power of exosomes to stimulate hair growth and restore confidence. This non-surgical approach is gaining popularity for its ability to address hair thinning and early-stage hair loss, particularly for conditions like androgenetic alopecia and non-scarring alopecias.

Exosomes are tiny extracellular vesicles (30–200 nm) that play a crucial role in cell-to-cell communication. They carry proteins, lipids, nucleic acids, and growth factors that promote tissue regeneration. In hair restoration, exosome therapy delivers these bioactive molecules directly to hair follicles, activating dormant follicles, prolonging the growth phase (anagen), and enhancing hair density and thickness. Research, including a 2024 South Korean study of 39 patients, showed significant improvements in hair density and thickness after 12 weeks of exosome treatment, highlighting its potential as a transformative therapy.

How Exosome Therapy Works

Exosome therapy involves applying a solution of exosomes to the scalp via microneedling, TED(ultrasound) or injections. These exosomes contain a high concentration of growth factors that stimulate hair follicle stem cells (HFSCs) and dermal papilla cells (DPCs). Key mechanisms include:

  • Promotion of Hair Follicle Growth: Exosomes activate HFSCs, initiating the anagen phase for new hair growth.
  • Stimulation of Angiogenesis: They promote new blood vessel formation, increasing nutrient supply to follicles.
  • Regulation of Hair Growth Cycle: Exosomes extend the growth phase and reduce premature shedding.
  • Anti-Inflammatory Effects: They reduce scalp inflammation, creating a favorable environment for hair growth.

The procedure, typically lasting 30-60 minutes. At Ageless Medical Spa and Hair Restoration, we use high-quality, regulated exosome products to ensure safety and efficacy.

The exosomes we use SMART eXo is a non-lyophilized biologic that contains hundreds of billions of exosomes per vial. Unlike other products, there are no other additives or preservatives, only exosomes in sterile saline. The minimal processing by low passage and careful extraction protects the lipid barrier to ensure optimal preservation of the contents.

Where are they from?

They are derived from perinatal Wharton’s Jelly extracted from mesenchymal stem cells, which have primary applications to skin cells and hair follicles. These tissues are collected by FDA-approved tissue banks that follow strict guidelines and protocols to ethically and safely collect neonatal tissue.

How they are used?

The exosomes can be used topically following treatments like microneedling, laser and other energy-based treatments. We have two different strengths. They can both be used for skin and scalp rejuvenation, post-procedure, and in combination therapies for mild to moderate hair thinning. We also offer another with double the exosomes per vial, is a higher potency option— especially effective to support graft viability and in advanced hair loss protocols.

How do I know exosomes from these tissues are safe?

Once collected, the tissues undergo rigorous testing and screening to ensure safety and quality. The tissue banks test both the donor and the tissues for infectious diseases. Advanced laboratory methods like PCR (polymerase chain reaction) and serology are used to detect the presence of pathogens or contamination. Donors are also screened through medical history reviews and health assessments to rule out conditions that could affect the safety of the tissues.

How are exosomes different from PRP?

Platelet-Rich Plasma (PRP) is a regenerative treatment that uses the patient’s own blood to stimulate healing and promote cell regeneration. PRP is rich in platelets, which contain growth factors and proteins essential for tissue repair and regeneration. PRP is using your own growth factors present in your own blood to trigger a healing regenerative reaction. Exosomes use neonatal cells that contain super high levels of growth factors from a donor to trigger a healing regenerative reaction. PRP and Exosomes can be used separately or together. Both have been shown in studies to increase the number and thickness of the follicles on patients' scalp. There are some studies that show a complementary benefit when used together.

FREQUENTLY ASKED QUESTIONS

What are Exosomes?

Exosomes are small vesicles (30–200 nm) secreted by cells, acting as messengers that deliver proteins, lipids, nucleic acids, and growth factors to influence cell behavior. In hair restoration, they stimulate hair follicles to promote the growth of more and thicker hair follicles as well as general scalp health.

How Does Exosome Therapy Work for Hair Loss?

Exosome therapy delivers concentrated growth factors to the scalp, activating dormant hair follicles and extending the growth phase. This leads to increased hair density and thickness, and can be particularly effective for androgenetic alopecia and non-scarring alopecias.

Is Exosome Therapy Safe?

Exosome therapy is generally safe, with minimal side effects reported in clinical studies. A 2025 review of 125 patients found rare side effects. At Ageless Medical Spa, we use bio ethically sourced, FDA-compliant exosomes to ensure safety.

What is the Cost of Exosome Therapy?

$400 to $1200 depending on the treatment area, is it an add to another procedure and how we are delivering the Exosomes to the follicles. We can deliver the exosomes by microneedling, TED(ultrasound) or injecting it into your scalp. Every patient is different and some delivery methods make more sense for certain patients. All this will be reviewed with you at your free consultation.

How Effective is Exosome Therapy Compared to Other Treatments?

Exosomes have been shown in published studies to increase the number and thickness of follicles on patients' scalps. Not every treatment works for every patient, we have no idea if any treatment will work or how it will work for any given patient. Exosomes are like a lot of other treatments that we offer.(Microneedling, LEDs, PRP, TED) the majority 75%ish of patients will have a measurable positive result (more or thicker follicles), but 25% will not have a measurable positive result. Some patients respond positively to all the different treatments, some patients don’t respond to any treatments. Studies show and anecdotally we have seen when you combine different proven treatments you tend to get a better result then just doing a stand alone treatment. That is why we really like to add exosomes to other treatments, it gives us a better chance of getting more of your hair back.. A small trial showed 51.6% hair loss reversal with exosome-PRP combination versus 21.5% with PRP alone.

How Many Sessions Are Needed, and When Can I See Results?

Most patients require 2–3 sessions, spaced 4–8 weeks apart. Initial results, such as reduced shedding, may appear within 4–12 weeks, with full results visible at 6 months, varying by individual response. We have a couple of different AI cameras to measure the number and thickness of the follicles on your head so we can track your results. We want to know where and how much it helped. For example you got 17% improvement at the crown, a 35% improvement at the vertex but only 5% on your temples.

Are There Any Side Effects?

Depending on the delivery method, side effects are uncommon and usually mild, including temporary redness, swelling, or discomfort at the injection site, typically resolving within days. Your scalp could be tender for 6-24 hours after microneedling. TED(ultrasound) slight soreness. Following post-treatment care, such as avoiding scalp washing for 24 hours, minimizes risks.

Who is a Good Candidate for Exosome Therapy?

Exosome therapy is ideal for individuals with early hair thinning or non-scarring alopecia, especially those who haven’t responded to treatments like minoxidil or PRP. It’s suitable for both men and women seeking a non-surgical solution. We offer a free consultation to review your goals and medical history to see if this is a good option for you.
